Report of Medical Examination
DD Form 2808 is the physician's side of the MEPS medical examination. After vision, hearing, blood pressure, urinalysis, blood draw, and the head-to-toe physical, the doctor completes the 2808 and assigns your PULHES profile. It becomes part of your permanent military medical record.
