Questionnaire for National Security Positions
SF-86 (electronically submitted via eQIP / e-QIP, which is being replaced by an updated system called eApp) is a comprehensive background investigation form. It covers residences, employment, education, foreign contacts, financial issues, drug history, alcohol use, mental health treatment, and police record over 7–10 years. Honesty is essential: investigators verify answers, and false statements are federal crimes.
